Community Health Survey

Community Health Assessment

In order to understand the current health needs of Jefferson County residents and identify where and how health can be improved, CHIP (Public Health’s Community Health Improvement Plan) is conducting a comprehensive health assessment of the community. In addition to the data gathering that we completed for the 2014 survey, we are gathering a diversity of voices from the community using an online (and paper) health survey and three public conversations (forums) around the county.

Friday, May 3.  11:30 am to 1:30 pm.  Free lunch provided.

  • Chimacum: Fire Station, 9193 Rhody Drive, Chimacum

Wednesday, May 15, 6:30 pm to 8:00 pm.  Refreshments provided.

  • Quilcene: Quilcene Community Center, 294952 US 101, Quilcene

Wednesday, May 29.  6:30 pm to 8:00 pm.  Refreshments provided.

 

The outcomes of this community outreach will inform health priorities in Jefferson County for the next

4-5 years. We would like to encourage you to take 5-10 minutes to complete the survey to help us

better understand the issues about health affecting Jefferson County. Please also consider attending one of our community conversations.
