Form 1095-B Notice:

The Treasury Department has determined that because the individual shared responsibility for medical payments was reduced to zero in 2019, an individual no longer needs the information on the Form 1095-B. For that reason, Chimacum School District will not be sending these forms out.  However, if you would like a Form 1095-B, you may request one from the Payroll Department. Please allow up to 30 days for processing.
