Student Health Services
The Chimacum School District employs a full-time ESA Certificated School Nurse to help keep students safe and healthy. The school nurse assists students and families with chronic health conditions; coordinates care for chronic, acute, and emergent medical issues; helps ensure the District’s compliance with state-mandated health programs such as vision and hearing screenings and immunizations; and serves as a resources to students, staff, and families.

When to Keep Sick Children Home From School: Deciding when to keep a sick child at home from school is not always easy. We realize decisions about guardian time off from work and childcare come into play, but when a child is ill, they need to stay home in the care of an adult to get well, decrease the chance of even longer, lingering illness, and to prevent spreading illness to others.
Vision and Hearing Screening
Good vision and hearing are important for academic success. Washington State law (WAC 246-760) requires schools to conduct auditory and visual screenings of children in kindergarten and grades 1, 2, 3, 5 and 7. “Passing” this basic screening does not guarantee a child is free of vision or hearing problems.
A referral letter recommending further professional evaluation is mailed to the parent/guardian, at the address which we have on file, of students who do not “pass” hearing and/or vision screening. A report from further evaluation is requested so that recommendations and/or accommodations can be made for students who need them. Students in grades 4, 6, 8 and 9 are screened if they receive Special Education Services or a screening is requested by a guardian or teacher.
Immunization Information
Washington State law requires immunizations for school entry. The laws can be found here: WAC Chapter 246-105 The school may ONLY accept immunization records on a Certificate of Immunization Status (CIS) Form. Please see the link below "How to access your family's immunization records" and look under "Required Documents" on that web page for more information. Medications at School
Whenever possible, medications should be given outside of school hours. For those needing prescription or over-the-counter medication, such as Tylenol, at school, an Authorization for Administration of Medications at School form must be completed by a guardian and medical provider and returned to the school. Authorization forms are valid for the current school year only and must be signed by a guardian and prescribing health care provider before medication can be dispensed to students.
Students may not carry any medications, prescription or over-the-counter, at school unless a medication authorization has been completed and self-carry has been authorized by the guardian and health care provider, and the student's knowledge has been assessed and student checked off by the School Nurse.
Medications not authorized for student self-carry must be brought to school and taken home by an adult and be in the original pharmacy container showing the student’s name and dosage. The label must match the prescription. If you have any questions about medication at school, please contact the school office or Nurse Reina.

Self-Administration of Asthma and Anaphylaxis Medications
It is the policy of the Chimacum School District that students with asthma or anaphylaxis are authorized, in consultation with the school’s health care consultant, to possess and self-administer medication for asthma or anaphylaxis during the school day, during school sponsored events or while traveling to and from school or school sponsored activities if the following conditions are met. Forms for Diabetic Students
Engrossed Substitute Senate Bill (ESSB) 6641 requires schools to prepare an individual health plan for each diabetic student and update it annually. In order to accomplish this for our diabetic students, we ask that you complete the following forms:
